"The Fibroid Book 2nd Edition": The purpose of this book is to enable women to become a doctor’s partner in an important aspect of their reproductive health, the treatment of fibroids of the uterus.
Chapter 1: What Is A Fibroid? & Why Should I Care? In this chapter the following questions are covered: Where do fibroids come from?, Who gets fibroids?,
The Inheritance of Fibroids? Fibroid tumor photos/illustrations are shown and explained. The diagrams show/name each section of the uterus and explains how a single cell causes tumors. Chapter 2: How Do I Know
I Have Fibroids? This section discusses 'abdominal protrusion', and has a photo/figure showing a patient with the medical problem. Diagnosing fibroid tumors are covered: Pelvic Ultrasound (i.e. Sonogram), Magnetic Resonance Imaging (MRI), Diagnostic Hysteroscopy, CT Scan, and Biopsy. An internal view/photo of submucus fibroids is shown. Chapter 3: Fibroids: Pregnancy & Contraception. Fibroids, pregnancy, and contraception are explained. Pregnancy complications associated with fibroids may be: Bleeding, fibroid degeneration, premature labor, premature delivery, blockage of the birth canal causing c-section, and growth retardation of the infant. At what point should myomectomy be performed in order to protect future fertility in a woman who is not currently nor has in the past attempted to get pregnant? Stories/examples of couples who have experienced pregnancy and fibroid issues are discussed. In this chapter the unique issues of women over 40 are listed and explained: No longer desire pregnancy, returning to careers after child rearing hiatus, and menopause is fast approaching. The period from forty to fifty years of age can be described as the “period of procrastination” for fibroids (menopause and hormone therapy are discussed). Chapter 5: So I Have Fibroids. What Are My Treatment Options? When considering treatment for uterine fibroids, I always like to emphasize to my patients that they need to strongly ask themselves, “Why do I need any treatment at all?.” 'Hutchins' First Law - Whenever you are considering surgery always make sure you have a good answer to the question: "How am I going to be better off?" In this section the following 'treatments' are covered: Drug treatment, Birth Control Pills, Progesterone, GnRH Analogues, and RU 486. Surgery: Hysterectomy, Myomectomy, Endometrial Ablation, and Myomacoagulation (i.e. Myolysis). Chapter 6: A New Alternative: Uterine Artery Embolization
The history of UAE, which goes back to the late 1980's is explained. In 1996, Dr. Hutchins and Dr. Worthington-Kirsch became the second team in the United States to offer UAE as a therapy for fibroid disease.
UAE illustrations of the procedure are shown. Chapter 7: Why Should I Be Treated? Appropriate indications for treating fibroids are discussed: Infertility, pressure or pain, blockage of the intestinal or urinary tract, and heavy bleeding. Misconceptions about when to treat fibroids are the following: Rapid change in the size of the uterus, inability to evaluate the ovaries, treatment based on size alone, and estrogen replacement. Chapter 8: Let's Talk: A Dialogue With a Female Gynecologist. Beverly Vaughn, M.D., FACOG, covers the topic:

I am strong, and What about the patient who declines our recommendation and insists on what is less than optimal treatment for her? The doctor patient relationship is complex. We both have to remember to ask questions and listen to the answers. This relationship must be built on trust or it will never work. Chapter 9: The Second Opinion. The informed and responsible individual will always seek to expand his or her knowledge, seeking additional consultation whenever faced with critical, medical conditions. This is not necessarily because of a lack of confidence in the doctor but out of recognition of the importance of the decision. Case studies describing a patient and their medical situations are explained in depth. Each case study has a section on "Analysis", which describes the problems. "Lessons" are listed after the Analysis to help the patient. The future of "Fibroidology" is explained. The following Therapies are explained: Gene Therapy, Ablative Therapy, Chronic Drug Therapy, and Uterine Artery Occlusion.
